Your personal information will never be sold, shared, or divulged with anyone without your written consent UNLESS you are an immediate danger to yourself or someone else.  All of your information is held in the strictest of confidence and in accordance with HIPAA and AMA guidelines. 

A signed Privacy Agreement will be provided prior to commencement of services.
